Lafayette County State Fair Forms

 

State Fair Dairy Exhibitor Information: All junior dairy exhibitors are required to attend a mandatory State Fair Dairy Orientation on Sunday, June 26 at 7:00 PM at the UW-Extension Office. Topics to be discussed at the meeting are: transportation, numbers of youth and animals, alternate animals, and supplies and expenses. We invite and encourage families to come with their child(ren) to the meeting. We will go over information that pertains to parents and exhibitors. If an exhibitor is not able to attend, a family member must be present.  ALL paperwork is DUE at the orientation meeting!

State Fair Livestock Exhibitors Information: All junior livestock exhibitors are required to attend a mandatory State Fair Livestock Orientation on Tuesday, June 28 at 7:00 PM at the UW-Extension Office. At the orientation, we will cover new changes at State Fair, guidelines, and procedures. We invite and encourage families to come with their child(ren) to the meeting.  If an exhibitor is not able to attend, a family member must be present.  ALL paperwork is DUE at the orientation meeting!

ATTENTION SWINE EXHIBITORS! All junior swine exhibitors must complete the Meat Animal Quality Assurance (MAQA) training or test (if you are eligible).  We will have one more class for MAQA— on Tuesday June 28 from 5-7 PM.  YOU MUST ATTEND THE CLASS IF YOU HAVE NOT TESTED OUT.

  • Youth Center Registration Form and Off Grounds Lodging Waiver Form
  • Wisconsin State Fair Youth Expectation Statement
  • Copy of Registration Form (Junior Dairy and Junior Swine breeding stock exhibitors)
  • State Fair Entry Form
  • Animal Registration Form
  • State Fair W-9 Form
  • Lafayette County Behavior Agreement (available on our website at and click on “State Fair information”)
  • State Fair Dairy Exhibitors Expectation Statement (for dairy exhibitors only-also available on our website-listed above)
  • Milk Sales Form (Junior Dairy Exhibitors only)
  • If you are staying in the dorms, you must complete the health form
